We provide medically tailored groceries and prepared meals, dietary supplements, and comprehensive nutrition services throughout all of Sonoma County.

- HIV Nutrition Program
Medically tailored weekly groceries for individuals with a positive HIV diagnosis, provided for as long as needed. - Welcome Home Program
Three months of medically tailored groceries and/or meals following an in-patient hospitalization. - Infectious Disease Program
Medically tailored groceries delivered weekly, with duration determined on a case-by-case basis. - Oncology Nutrition Program
Three months of medically tailored groceries or meals for the client and their household. Service duration may be extended based on the course of treatment. - Full Belly Program
Medically tailored groceries and meals for expecting clients and their household, continuing until the baby reaches three months of age. - Community Supports Program
Twelve weeks of medically tailored groceries and meals for patients with Medi-Cal, with medical approval. - Bags of Love
Ready-to-eat meals distributed to service providers for their unhoused patients.

Welcome Home Program
Who’s Eligible | Patients being discharged from an in-patient hospital or mental health hospital stay within two weeks of their discharge date. Must score 6 or higher on DETERMINE Nutrition Checklist (screening is on back of the referral form) |

Oncology Nutrition Program
Who’s Eligible | Patients undergoing cancer treatment and members of their household |

Full Belly Program
Who’s Eligible | Patients who are pregnant and at risk for malnutrition and members of their household |

Program Details | Length of pregnancy and for 12 weeks after the baby is born |

Infectious Disease Program
Who’s Eligible | Patients with a positive COVID-19 test, Tuberculosis, or Monkeypox diagnosis or patients who are debilitated by Long COVID |
Services Provided | Free weekly groceries, including fresh produce, high-quality protein, healthy grains, vitamins and supplements Medically tailored prepared meals, if needed Home delivery, if needed Individual nutrition counseling |
Program Details | Varies by case. Food For Thought’s client services team will be in touch with patients on a weekly basis to determine program length |

Community Supports (ILOS) Program
Examples of Who’s Eligible | Patients with Medi-Cal and medical justification for medically tailored groceries or meals. A medical justification is required. Examples include, but are not limited to: A client with poorly controlled diabetes who can benefit from diabetes friendly groceries to help stabilize blood sugars and get used to eating foods that will help their health A client with issues that make it hard for them to shop, e.g. physical disabilities, etc, while also coping with GI issues that require specific foods or diets |

Bags of Love
Who’s Eligible | Nutrition for your patients experiencing homelessness distributed by local health centers |
Services Provided | Our Bags of Love program provides ready-to-eat food to patients who are experiencing homelessness and serious mental and/or physical health conditions. To streamline the distribution process, we partner with local health centers who work directly with people without housing. These organizations distribute the bags of food onsite at their clinics and offices. Each bag of food has the caloric value of 11 meals |
