We provide medically tailored groceries and prepared meals, dietary supplements, and comprehensive nutrition services throughout all of Sonoma County.

- HIV Nutrition Program
Medically tailored weekly groceries for individuals with a positive HIV diagnosis, provided for as long as needed. - Welcome Home Program
12 weeks of medically tailored groceries and/or meals following an inpatient stay. - Oncology Nutrition Program
12 weeks of medically tailored groceries or meals for the client and their household. - Full Belly Program
Medically tailored groceries and meals for expecting clients and their household, continuing for 12 weeks after baby is born. - Community Supports Program
12 weeks of medically tailored groceries and meals for patients with Partnership HealthPlan insurance, with medical approval. - Infectious Disease Program
Medically tailored groceries delivered weekly, with duration determined on a case-by-case basis.

HIV Nutrition Program

Welcome Home Program
| Who’s Eligible | Sonoma County residents with chronic medical conditions who were recently discharged from an inpatient facility, such as hospital, skilled nursing, psychiatric, or substance use treatment |
| Services Provided | Free weekly groceries, including fresh produce, high-quality protein, and healthy grains Medically tailored prepared meals, if needed Home delivery, if needed Individual nutrition counseling |
| Program Details | 12 consecutive weeks |

Oncology Nutrition Program
| Who’s Eligible | Sonoma County residents who are actively undergoing chemotherapy or immunotherapy for cancer treatment |
| Services Provided | Free weekly groceries, including fresh produce, high-quality protein, and healthy grains Medically tailored prepared meals, if needed Home delivery, if needed Individual nutrition counseling Nutrition supplements, such as Ensure and protein powder, if needed |
| Program Details | 12 consecutive weeks |

Full Belly Program
| Who’s Eligible | Patients who are pregnant and at risk for malnutrition, plus household members |
| Services Provided | Free weekly groceries, including fresh produce, high-quality protein, and healthy grains Medically tailored prepared meals, if needed Home delivery, if needed Individual nutrition counseling |
| Program Details | Length of pregnancy and for 12 weeks after the baby is born |

Community Supports (ILOS) Program
| Who’s Eligible | Sonoma County residents with Partnership HealthPlan insurance who have need for medically tailored groceries or meals Examples include, but are not limited to: Someone with diabetes who could benefit from diabetes-friendly groceries to help stabilize blood sugars Someone facing issues that make it hard to shop, while also facing GI issues that require specific foods |
| Services Provided | Free weekly groceries, including fresh produce, high-quality protein, and healthy grains Medically tailored prepared meals, if needed Home delivery, if needed Individual nutrition counseling |
| Program Details | 12 weeks Possibility of a one-time extension depending on need and approval by Partnership |

Infectious Disease Program
| Who’s Eligible | Patients with a positive COVID-19 test, Tuberculosis, or Monkeypox diagnosis or patients who are debilitated by Long COVID |
| Services Provided | Free weekly groceries, including fresh produce, high-quality protein, and healthy grains Medically tailored prepared meals, if needed Home delivery, if needed Individual nutrition counseling |
| Program Details | Varies by case. Food For Thought’s client services team will be in touch with patients on a weekly basis to determine program length |
